Order Form

If you would prefer to pay by cheque or postal order, you will need to fill in an order form.

1) Either, download a pdf of the order form and print it out,

OR

download a Word document of the order form

2) Then fill in the form, writing the quantity of each item in the last column.

3) Add up the total of the items you want and write your cheque for this amount.

4) Fill in your name and address details.

5) Post the completed order form and your cheque to us at People First.
